I further understand that the Collin County Criminal District Attorney will be conducting a thorough background investigation that may include, but is not limited to, criminal history, employment history, and personal references. I declare that I have never been convicted of a felony offense in this state, any other state, or the United States. I also understand that any participant may be removed from the Collin County Criminal District Attorney’s Internship Program if said participant is disruptive or otherwise inhibits the concept or implementation of this program.

CONSENT FOR CRIMINAL BACKGROUND CHECK: The Collin County District Attorney's office must perform criminal background checks on Internship Program applicants because of the matters of the population we serve. Please read and sign this consent form authorizing the Collin County District Attorney's office to perform a criminal background check.

I hereby give my permission for the Collin County District Attorney's office to obtain information relation to my criminal history record. The criminal history record, as received from reporting agencies, may include arrest and conviction data as well as plea bargains and deferred adjudication dispositions and delinquent conduct committed as a juvenile. I understand that this information will be used, in part, to determine my eligibility for being an intern with this organization. I also understand that as long as I remain an intern with the Collin County District Attorney's office, the criminal history records check may be repeated at any time.
